• We are contracted and credentialed with most commercial insurance companies, as well as Medicare and Oregon Health Plan.
• If a patient’s plan does not cover medical nutrition therapy for their diagnosis, we give them a 40% discount for self-pay at time of service.
• We accept major credit cards (except for American Express), and Health Savings Account credit cards.

FAX referrals to 541-312-4670 and include:

• Patient’s name
• Contact information
• Nutrition diagnosis (e.g. diabetes, celiac disease, over/underweight)
• Any relevant health information and lab values
